It’s always a good idea to review your UnitedHealthcare plan details before booking. UnitedHealthcare (UHC) directories may show me under certain products, but eligibility and coverage depend on individual plan details. I am in-network with many commercial Choice plans for preventative lactation care. Important: EPO plans list me as in-network but won’t pay for my lactation service code S9443 from my NPI, so those visits are self-pay, payment collected upfront. - Book Your Appointment:
