Self-pay may be the right fit if you don't have insurance, if I'm out of network with your plan, or if you'd simply rather work with me directly without going through an insurer's process.

If your insurance plan offers out-of-network benefits, I can provide a superbill (an itemized receipt) for you to submit to your insurance company on your own. Your insurance company determines whether and how much it will reimburse, and reimbursement is not guaranteed. I don't handle insurance authorization, submit claims, or negotiate with your insurer on your behalf.

AETNA IN-NETWORK LACTATION VISITS

IBCLC Lactation Consultant

I'm in-network with most Aetna commercial plans for lactation services.


Aetna's lactation benefit can be more complicated than simply saying "six lactation visits." Many plans use S9443 units, and services for both a parent and baby may draw from the same family-level benefit.
For example, if S9443 is billed for both you and your baby, six available units may provide approximately three visits in which both are billed.
Other providers including OB/GYNs, pediatricians, hospitals, and lactation providers may also have billed against the same benefit.
Before your appointment, ask Aetna:

• How many S9443 units have been used during this plan year?
• How many units remain?
• Which providers have billed S9443?
• Are lactation services covered at 100% under my specific plan?
• Are there deductible, copay, or coinsurance requirements?

Because neither you nor I can see Aetna's internal benefit tracking in real time, checking your benefits does not guarantee final payment.
I will bill Aetna when I am able to do so. If Aetna determines that your benefits are exhausted or assigns an amount to you, that amount is your responsibility.

Some Anthem members may have access to $0 virtual lactation support through Wildflower Health through the Building Healthy Families benefit.
This benefit is not available with every Anthem plan and is virtual only.
Eligible members may receive:

• Up to 2 virtual prenatal lactation visits
• Up to 6 virtual postpartum lactation visits
• Additional visits when prior approval is obtained

HORIZON BCBS + ANTHEM (in office) + OTHER OUT-OF-NETWORK PLANS

I am out of network with Horizon Blue Cross Blue Shield because Horizon does not credential IBCLCs in New Jersey. Same with Anthem for office visits, though Anthem does offer virtual visits to eligible members.
If your insurance plan has out-of-network benefits for lactation services, you may pay for your visit directly and submit a superbill that I supply to for possible reimbursement to your insurance company.
Your insurance company determines whether the service is eligible for reimbursement and how much it will reimburse. I cannot guarantee reimbursement.

WHAT DOES “COVERED” MEAN?

"Covered" does not necessarily mean that your visit will be paid in full.Depending on your plan, you may have a deductible, copayment, coinsurance, benefit limitation, authorization requirement, or other cost-sharing responsibility.
